Requirements for Participation in Athletics

All students are required to have on file in the Athletic Trainer's Office copies of the following documents. These are updated annually. They are:

Physical Examination Form
Acknowledgement of the Rules Form
Emergency Information/Consent to Treatment Form
Release of Information Authorization Form
Insurance Information
Steroid Information

Athletic insurance is offered to all students who are participating in UIL sponsored activities. This is a secondary policy that will pay claims after they have been filed with the athlete's primary insurance provider. Policy requirements are subject to change and do have limitations.
